COVID-19 WELLNESS ASSESSMENT
People with Covid-19 have had a wide range of symptoms, ranging from mild symptoms to severe illness. Symptoms may appear 2-14 days after exposure to the virus. People with cough, chest or nasal congestion, shortness of breath or difficulty breathing, fever, chills, repeated shaking, body aches or muscle pain may have Covid-19. The virus is spread through respiratory droplets produced when an infected person coughs or sneezes. These droplets can land in the mouths or noses of people who are nearby or possibly be inhaled into the lungs.
Our staff will come to your location and make an initial impression in a Primary Survey. This includes a visual assessment and we will form an objective opinion of the persons general wellbeing. If person presents
with any abnormalities our staff will perform a complete patient assessment. We will incorporate Covid-19 screener questions into the initial impression.
As part of a Secondary Survey our staff will take the persons temperature using
a temporal thermometer
If needed a complete Patient Assessment will be completed. A full
assessment will be performed and be recorded on an Electronic Patient Care Report. Included in the Patient Assessment are all vital signs.
If any symptoms present during the complete patient assessment the person will be referred to a health care facility for further evaluation. As part of the Medical Referral we will:
- Provide a face mask for the person to wear over their nose and mouth.
- Direct them to the area designated for symptomatic persons.
- Wellness Assessment Forms can be recorded and provided.
